Realizing Faith at Trinity Health Center
By spreading Christ’s love through quality healthcare to those in need, Trinity Health Center provides hope and healing to our underserved neighbors in DeSoto County.
Faith Community Engagement
Trinity Health Center believes in the value Faith Community Nurses bring to the underserved population and plans to enlist FCNs in partnering with existing medical and social service providers. FCN helps assess needs patients have and help connect them to resources that can help them. Trinity Health Center has built relationships with community agencies and medical providers that will benefit the work of FCNs.
Trinity Health Center plans to employ a social worker that will help patients in those areas, besides healthcare, that impact their life. For many, putting food on their table or paying the rent comes before taking care of their health. The intervention of a social worker and coordinating with Faith Community Nurses has shown dramatic savings to hospital systems through decreasing emergency room usage. These interventions led to an increasing number of emergency room users converting to primary care providers and having better-controlled diabetes and improvement in overall quality of life.

How Faith Communities can support Trinity Health Center
Local DeSoto County churches founded Trinity Health Center in collaboration and with a vision for the medical clinic to be a way for all the churches and houses of worship in DeSoto County to come together to serve our community. There are several ways for churches to get involved with Trinity Health Center.
